Tissue bioengineering and regenerative aesthetic medicine have emerged as rapidly evolving interdisciplinary fields that integrate plastic surgery, regenerative medicine, endocrinology, molecular biology, biomaterials science, and biomedical engineering to promote tissue restoration and functional recovery. The objective of this review was to analyze current advances in regenerative technologies and their applications in aesthetic and reconstructive medicine. A comprehensive review of scientific literature indexed in major biomedical databases was conducted, focusing on stem cell therapies, platelet-rich plasma, extracellular vesicles, bioactive scaffolds, biomaterials, and three-dimensional bioprinting. The analyzed evidence demonstrated that adipose-derived stem cells and platelet-rich plasma remain the most established regenerative interventions due to their favorable safety profiles, accessibility, and demonstrated effectiveness in tissue repair and rejuvenation. Emerging technologies such as exosome-based therapies and advanced tissue-engineering platforms exhibited significant innovation potential and may contribute to future therapeutic developments. Angiogenesis, extracellular matrix remodeling, cellular signaling, and endocrine regulation were identified as essential mechanisms underlying successful regenerative outcomes. Furthermore, the findings highlighted the increasing importance of personalized regenerative approaches that incorporate metabolic and hormonal optimization. Despite substantial progress, challenges related to protocol standardization, long-term clinical validation, regulatory frameworks, and accessibility remain significant barriers to widespread implementation. Overall, tissue bioengineering and regenerative aesthetic medicine represent promising strategies capable of transforming contemporary healthcare by promoting biological restoration, improving aesthetic outcomes, and advancing personalized therapeutic interventions.
